I am a 68 year old male who has diabetes.  Prior to receiving a kidney in 2008, I was on peritoneal dialysis for four and a half years.  As a result, the muscles of my stomach were stretched and now my stomach is extremely large.  How can I get rid of this protruding stomach?

Hi thank you for asking the question. 
The only thing that can help, is exercise, your muscle are stretched and it is difficult to decrease the length of the muscle. Considering your age, I would suggest continuing with exercise as tolerated with the goal to increase the strength of your abdominal muscles. No medicine is available that can help to decrease the length of the stretched muscles. due to your kidney issue, you may not be allowed to take a diet high in protein, due to the risk of progression of your kidney disease. You should also think about abdominal binders that may help to reduce you abdominal size cosmetically. 
If it is abdominal fat then again aerobic exercise, such as walking, and dietary control (restricted calorie intake, while avoiding hypoglycemia) is advised.
